If you selected Alliance Network, Double Network or Network Choice as your option, it is necessary that you nominate two GP’s for you and each of your beneficiaries, from the CAMAF Network. You are required to use your nominated GP’s for all your GP consultations.
Please note you are entitled to change your nominated GP’s once annually so please consider carefully who your GP will be, along with an alternate, should your GP be unavailable.
You may search for your GP using the Geomapping facility.
Kindly email your nominated GP selection for each beneficiary to [email protected] and allow 48 hours for the update to reflect. It is essential that you include the GP practice name and practice number, your name and membership number as well as your beneficiary’s name(s)
